Have the BIN rules changed?

IIRC, at one time, if someone placed a bid on an item with a BIN option, the
BIN option was removed after the first bid.

Does the BIN now stay in place, even after the first bid?

Only if the first bid is below the reserve price, AFAIK the BIN will only
disappear once the reserve price is met.

Ah, that explains it. There was a reserve on the item which hadn't been
reached...
